On post-stroke depression and Purple Star

A short note on a pattern that has been under-recognized in one language and named for centuries in another. The Western label is post-stroke depression. The classical TCM read is older, and more specific.

The clinical picture.

A patient sits down across from you six months after her stroke. Her physical recovery is on track. Her physical therapist is satisfied. Her neurologist is satisfied. She is not. The light has gone out of her. The things she used to enjoy no longer reach her. Sleep is heavy but not restorative. She tells you, quietly, that she does not feel like herself.

The TCM pattern.

Liver qi bound with Spleen depletion — the pattern the classics wrote for the patient whose qi has knotted at the middle and cannot move. In the post-stroke body this pattern is amplified: the injury has disturbed the axis between the digestion and the mood at both ends. The texts named this as Liver-Spleen disharmony nine hundred years before the modern research described the gut-inflammation-brain path underneath it.

Eight herbs — Chai Hu and Bo He to release the constrained Liver, Dang Gui and Bai Shao to build the blood, Bai Zhu and Fu Ling to strengthen the Spleen, ginger and Gan Cao to harmonize. What the modern research is filling in.

Published work on Xiao Yao San in post-stroke depression has documented an effect on the gut microbiome and on the inflammatory signals that travel from gut to brain — the classical Liver-Spleen relationship described from the other side. In the trial the formula reached the depression at the level a standard antidepressant reached it.

What I recommend.

For the patient who has come back from the stroke but not back to herself — Purple Star at two bags daily for the first month, then one bag daily for months two and three. Combine with daily Microgard for the gut ground the same path answers to. The psychiatric care continues alongside. Re-evaluate mood, sleep, and enjoyment at the three-month mark.
